Lombardia L.R. 13/2021 requires 300 hours of training for piercers (and 1,000+ hours for combined practitioners), SCIA with the Comune, a Class B autoclave for reusable instruments, a dedicated sterilization area, and written informed consent. Toscana L.R. 28/2004 and Lazio L.R. 2/2021 follow similar structures with their own training thresholds and penalty schedules. The ASL/ATS conducts announced and unannounced inspections. In Lombardia, fines range from €3,000 to €15,000 with suspension or closure.
